Sterling Vision has partnered with Sharecare to securely process all patient medical records through a convenient self-service portal. Below, you’ll find instructions for submitting a request, along with helpful resources including a QR code that allows you to request your records from a mobile device, links to check status on your request, printable forms, fax details, and in-person drop-off options, plus so much more! An invoice will be prepared directly from Sharecare for all applicable medical record request costs. You will not pay the practice. For Patients
Need a copy of your medical records for your own use or to share with another provider? You can submit your request quickly through our secure Sharecare portal. It’s fast, easy, and there’s no need to call our clinic directly.
Third Parties
Requesting records on behalf of a patient (attorneys, insurers, case managers, disability paperwork, etc.)? Please use the link below to submit your request through our secure Sharecare portal. Requests without authorization will not be processed. Please have those documents ready before submission.
